Enrollment Confirmation

Please complete the form below to accept, decline, or defer your enrollment. By confirming your enrollment and paying your enrollment deposit, you will secure your place as a student of Davis College. 

The enrollment deposit is a down payment on your tuition. It is nonrefundable from the time it is submitted. Failure to enroll for the admitted term will result in forfeiture of the deposit.
