Transcript Request Form

A permanent record of grades is maintained for each student by the Registrar's Office. Students can order official and unofficial transcripts by completing and submitting the form below. Official transcripts are sent by mail. Unofficial transcripts are sent by email or fax.

  • Requests may also be submitted by fax to 607.797.5773. Faxed requests must be signed by the student and include the student's social security number. 
  • Transcripts will not be issued for anyone with outstanding financial obligations to the college.
  • A fee of $10.00 will be assessed for each transcript. If multiple transcripts are requested at the same time, the cost will be $5.00 for each additional transcript within the same request.
  • Transcripts will be processed within 15-20 business days of the receipt of both the completed request form and the fee. A request may be expedited and overnighted for an additional $50.00 if the fee is received by 11:00am. (Due to Covid-19 issues, transcript requests for expedited and overnighted Service is temporarily unavailable)
  • Official Transcripts cannot be faxed or emailed.

Many large schools--especially Liberty and SUNY schools--have specific address for transcripts. Please provide as specific an address as possible or your transcript may be lost at the receiving institution.
