Transcript Request Form

A permanent record of grades is maintained for each student by the Registrar's Office. Students can order official and unofficial transcripts by completing and submitting the form below. Official transcripts are sent by mail. Unofficial transcripts are sent by email or fax.

  • Requests may also be submitted by fax to (888) 343-9196. Faxed requests must be signed by the student and include the student's social security number. 
  • A fee of $10.00 will be assessed for each transcript. If multiple transcripts are requested at the same time, the cost will be $5.00 for each additional transcript within the same request.
  • Transcripts will be processed within 15-20 business days of the receipt of both the completed request form and the fee. A request may be expedited and overnighted for an additional $50.00 if the fee is received by 11:00am. 
  • Official Transcripts cannot be faxed or emailed.

It does not give others permission to alter the student’s record. Changes to the student record must be made at the student’s request.
